3、Qualification Requirements For Bidder
Qualifications or Performance:1. If the goods provided by the bidder in accordance with the contract are not made by the bidder itself, the bidder shall obtain and provide a formal authorization letter from the goods manufacturer agreeing to provide the goods in this bid. (For the format, see the authorization letter issued by the manufacturer in Chapter 4)
2. The goods provided by the bidder shall be the equipment that has obtained the "Medical Device Registration Certificate" issued by the relevant department of the People's Republic of China and provide the certification materials;
3. The bidder shall provide the original or photocopy of the bank credit certificate issued by the bank that opened the basic account within three months before the bid opening.
4. Domestic bidders shall provide a copy of the qualified business license (copy) and a copy of the medical device production (operating) license.
5. The authorized representative of the domestic bidder shall attach the Authorization Letter of the Person in Charge of the Unit in writing to the bidding documents, and the power of attorney shall be affixed with the official seal of the bidder. If the legal representative himself participates in the bidding, this item is not required.
The bid evaluation committee will retain the verification authority for all the original qualification documents at the time of bid evaluation.
