1.中化商务有限公司（招标机构）受四川省科学城医院（招标人）委托，以国际招标方式邀请潜在投标人就下列货物和有关服务提交密封投标：
高清电子胃肠镜 1批（1台主机，2条胃镜，1条肠镜）
2.有兴趣的投标人可从中化商务有限公司得到进一步的信息和查阅招标文件。
3.有兴趣的投标人可从2020年06月04日起至2020年06月10日每天（节假日除外）北京时间上午9时至下午16时登录中化招标电子招投标平台（报名链接：e.sinochemitc.com；技术支持电话：010-86391277,010-59369333）报名并购买招标文件，本招标文件每包按300元/本出售，售后不退。任何未在招标人或招标机构处领购招标文件的法人或其他组织均不得参加投标。
4.所有投标文件应于2020年06月30日北京时间下午14:00之前递交到以下地点，开标仪式定于2020年06月30日北京时间下午14:00北京时间举行，届时请投标人代表出席开标仪式：

5.主要资格
①投标人提供有效的营业执照和税务登记（如适用）证明文件。
②投标人需提供投标机型的“国家医疗器械注册证”即SFDA认证，同时要获得FDA或CE认证，提供前述认证证书复印件。
③投标人提供近三年（2016-2018年或2017至2019年）经审计的审计报告或企业财务报表（资产负债表、现金流量表和损益表）复印件。
④境内投标人须提供银行基本账户开户许可证复印件或提供基本存款账户信息。
⑤投标人应无行贿犯罪记录，并提供书面声明（详见格式文件）。
6.业绩要求
投标人所投产品制造商或代理商提供的同类产品（主设备）应在中华人民共和国境内有3套及以上使用先例。投标人须提供用户联系方式，并提供项目合同或用户证明作为业绩证明材料。未提供有效业绩证明材料或业绩数量不满足前述要求的，评标价格将在投标价格基础上增加1%。

1.Entrusted by Sichuan Science City Hospital (Tenderee), SINOCHEM COMMERCE CO., LTD. (Tendering Agent) invites sealed bids by international (competitive) from eligible bidders for the supply of the following goods and related services:
High-definition electronic gastroscope Batch 1(1 mainframe, 2 gastroscopy, 1 colonoscopy)
2.Interested bidders may obtain further information and refer to Bidding Documents from SINOCHEM COMMERCE CO., LTD.
3.Interested bidders may purchase Bidding Documents through website(link:e.sinochemitc.com) from 4th,Jun, 2020 till 10th, Jun, 2020 during the period 09:00~16:00 (Beijing time) upon non-refundable payment of RMB 300.00 or USD42.21 for each complete set of bidding documents. Bidders that fail to fill out the purchase record of the Bidding Documents shall be rejected.
4.All Bids must be delivered to the following address before 14:00pm, 30st, Jun, 2020 (Beijing time). Time for bid opening ceremony is 14:00pm, 30st, Jun, 2020 (Beijing time), the representatives of the bidders are kindly requested to be present at the time.

5.Principal qualification
① The bidder shall provide valid business license and tax registration (if applicable) supporting documents.
②Bidders are required to provide the "national medical device registration certificate" (SFDA) for the bid model, and to obtain FDA or CE certification by providing a copy of the said certification.
③The bidder shall provide a copy of the audited audit report or the financial statements of the enterprise (balance sheet, cash flow statement and profit and loss statement) for the last three years (2016-2018 or 2017-2019) .
④Domestic bidders shall provide a copy of the bank basic account opening license or basic deposit account information.
⑤The bidder shall have no criminal record of offering bribes and shall provide a written statement (see the format document for details) .
6.Performance conditions
The same type of products (main equipment) supplied by the manufacturer or agent of the products to be put up by the bidder shall have 3 sets or more precedents for use in the People's Republic of China. Bidders are required to provide the customer contact information and the project contract or user certificate as performance evidence. The bid evaluation price will be increased by 1% over the bid price if no valid performance proof material is provided or the performance quantity does not meet the above requirements.
