3、Qualification Requirements For Bidder
Qualifications or Performance: I. The qualification documents submitted by the bidder to prove that the bidder can perform the contract after winning the bid shall include the following documents:
1) if the goods is not provided by the bidder in accordance with the contract make the bidder himself, the bidder shall be the goods manufacturers agreed to in the tender to provide the goods the effective power of attorney or agent with authorized access authorization for bidding product, and shall provide the agent with a valid authorization permissions related documents, documents need to show the goods manufacturers to bid product licensing chain integrity); When determining the number of bidders, if the bidding products of two or more bidders are produced by the same manufacturer or integrator, it shall be determined by one bidder. If two or more integrators or agents use the same manufacturer's products as part of their project package, and the total price of the same products exceeds 60% of the total bid price of the project package respectively, it will be determined by different bidders.
2) Power of attorney of the bidder's legal representative.
3) The original or copy of the credit certificate issued by the bank of account of the bidder within three months before the bid opening date (the copy shall be stamped with the effective seal or signed by the bidder).
2. "Other qualification documents" refer to the documents that prove that the bidder or the products he/she has submitted meet the following qualification or qualification requirements:
1. If the bidder is registered in China, the bidder shall provide a copy of the business license, organization code and tax registration certificate (for enterprises that have implemented the multi-certificate integration, provide a copy of the multi-certificate integration business license);
2. The bidding products manufactured outside the territory of the People's Republic of China shall have valid medical device registration certificate (including the registration form); The bidding products shall be the medical device manufacturing enterprise license and valid medical device registration certificate for the products manufactured within the territory of the People's Republic of China (including the registration form, and relevant valid acceptance notice shall be provided if the period of validity has expired); If the bidding products are subject to compulsory certification stipulated by the state, relevant certification certificates shall be provided; If the period of validity has expired, the relevant notice of acceptance shall also be provided;
3. Bidders should provide valid medical device business license for domestic agents;
4. The legal representative or the main person in charge of the bidder participating in the project shall not have the evidence of bribery criminal record within the first three years (provide the original letter of commitment).
