3、Qualification Requirements For Bidder
Qualifications or Performance:1. If the goods provided by the bidder in accordance with the contract are not manufactured by the bidder, the bidder shall obtain and provide the official authorization letter from the manufacturer of the goods.
2. The goods provided by the Bidder shall be the equipment with the "Medical Device Registration Certificate" issued by the relevant authorities of the People's Republic of China. Provide a copy of the complete medical device product registration certificate (if any attachment is provided).
3. The bidder shall provide the copy of the relevant certificate stipulated by other compulsory certification of the state. (if any)
The bidder shall provide the bank credit certificate issued by the bank that has opened the basic account within three months before the bid opening (with the bank opening permit or the basic account information certificate).
5. Domestic bidders shall provide the copy of qualified business license.
6. If the domestic bidder is the agent or distributor of the invested product, the copy of Business License of Medical Device shall be provided; If the domestic bidder is the manufacturer of the invested product, copies of Production License of Medical Device and Business License of Medical Device shall be provided.
7. The authorized representative of the domestic bidder shall attach the original "Letter of Authorization of Legal Representative" issued in written form and the copy of the ID card of the representative of the bidder to the bidding documents, and the Letter of Authorization shall be affixed with the official seal of the bidder. This is not required if the legal representative himself participates in the bidding.
The bidder shall guarantee that the response qualification is true and effective within the specified time limit.
If the above eligibility criteria are not met, the bid will be rejected.
The Bid Evaluation Committee will reserve the right to verify all original qualification documents at the time of bid evaluation.
