1.1provide the medical device registration certificate and its attachments or the valid copies of the medical device record proof materials according to the type of the medical device in the tender.
1.2 according to the type of the medical device in the tender and whether the bidder is a manufacturer, provide the valid copy of the medical device business license or medical device business record certificate or medical device production license.
2. Domestic bidders must provide the copy of legal person business license (duplicate) and organization code certificate with official seal. (if the business license loaded with the unified social credit code is provided, it shall be deemed that the organization code certificate has been provided).
