3、Qualification Requirements For Bidder
Qualifications or Performance:1. Those who provide class II and III medical devices shall have the medical device registration certificate, and those who provide Class I medical devices shall have the product record registration certificate;
2. Those who provide the second and third class medical devices produced in the territory of the People's Republic of China shall have the medical device production license, and the first class medical devices shall have the medical device production record certificate;
3. The operation of Class III medical devices shall have the medical device operating enterprise license, and the operation of Class II medical devices shall have the record registration certificate of the medical device trading enterprise (the medical device registrant, record holder or the manufacturer sells the medical devices at its domicile or production address, do not need to provide).
4. If the goods invested by the bidder are not manufactured by itself, the bidder shall obtain the consent of the manufacturer to provide the formal authorization letter or distribution authorization of the goods in this bid (applicable to imported products, if it is not necessary to provide for domestic products);
5. If the bidder is an enterprise or institution legal person in the People's Republic of China, it shall also provide valid business license, organization code certificate, tax registration certificate (or business license combining three certificates);
6. Consortium bidding is not accepted for this project;
