3、Qualification Requirements For Bidder
Qualifications or Performance:1. If the goods provided by the bidder under the contract are not manufactured by the bidder themselves, the bidder shall obtain and provide a formal authorization letter from the manufacturer of the goods.
2. The goods provided by the bidder shall be equipment that has obtained the "Medical Device Registration Certificate" issued by the relevant departments of the People's Republic of China. Provide a complete copy of the medical device product registration certificate (if attached) for the goods.
3. Bidders shall provide copies of relevant certificates required by other mandatory national certification regulations. if any
4. Bidders shall provide a valid bank credit certificate (with bank account opening permit or basic account information proof) issued by the bank where they opened their basic account within three months before the bid opening.
5. Domestic bidders shall provide a qualified copy of their business license.
If the domestic bidder is an agent or distributor of the product being bid on, they shall provide a copy of the Medical Device Business License; If the domestic bidder is the manufacturer of the product being bid on, they should provide copies of the "Medical Device Production License" and "Medical Device Business License". (The goods purchased in each procurement package of this project belong to Class II medical device products)
7. The authorized representative of the domestic bidder shall attach the original "Authorization Letter of Legal Representative" issued in writing and a copy of the bidder's representative's ID card to the bidding documents, and the authorization letter shall be stamped with the bidder's corporate seal. If the legal representative participates in the bidding themselves, this item is not required.
Bidders shall ensure that their response qualifications are genuine and valid within the prescribed period.
Those who do not meet the above qualification criteria will have their bids rejected.
The evaluation committee reserves the right to verify all original qualification documents during the evaluation process.
